Investigation Summary

Investigation Nr: 201085776
Event: 08/18/2004
Employee Injured When Finger Is Caught in Rotating Blade

At approximately 4:42 p.m. on August 18, 2004, Employee #1 was working as a machine operator in the paste grinding area for Chooljian Brothers Packing Company, Inc., who processes and packages raisins. It was the end of his shift and Employee #1 was using a water hose to rinse off residual raisin debris from a handling table. The raisins are delivered by a chute over the table, and are moved down into a screw conveyer (auger), for transport. The table is approximately waist high, and normally has a grate over the screw conveyer. The grate had been removed for unknown reasons. Employee #1 inadvertently stepped onto the wet hose and slipped. He fell forward and his left hand inadvertently went into the exposed trough containing the rotating screw conveyer. Employee #1's left middle finger was trapped between the conveyer blade and the wall of the trough. As a result, he suffered surgical amputation of finger to the joint between the proximal and middle phalanx bones.
