Investigation Summary
On September 6, 2003, Employee #1 was driving a standup forklift. The employee was in the main corridor between old and new warehouses, and stopped at the stop sign and tooted her horn before proceeding into the old warehouse. She saw a coworker also driving a standup forklift heading toward her and coming out of one of the aisles. The two forklifts collided. Employee #1's left foot was up and outside of the forklift and became pinned at the base of the forklift, causing a fractured left ankle. Employee #1 was hospitalized. The Employer is a distribution warehouse for stores in California and both employees were full-time employees of the employer and had received forklift training.
