Investigation Summary
Employee #1, a sanitation worker, was operating a pallet jack at a chicken processing plant. The pallet jack went into reverse causing his fingers to become jammed between the pallet jack and an electrical plug on the wall. His right ring finger was amputated at the first joint and he was hospitalized. The employee was not certified to operate an industrial truck.
