Employee's Little Finger Is Amputated in Conveyor Tray

At approximately 4:20 a.m. on August 5, 2004, Employee # 1 and a coworker just completed a sanitation procedure on a beverage pouch filler machine. In accordance with procedure, they started the filler machine and stationed themselves on either side of the water-filled cooling bath compartment. They were required to pull and discard the first sixty pouches produced. As Employee # 1 was reaching the pouch, his pinkie finger on his right hand was caught in the conveyor tray. Employee # 1 forcefully pulled back his right hand what resulted in a partial amputation of his pinkie finger. Additional surgical amputation to the first joint of Employee's right little finger was required.
