Investigation Summary
At approximately 9:20 a.m. on June 3, 2004, an employee was moving the curtain to the left of the door and was placing the curtain in a guarded hole in the wall, where the cross belt conveyor belt and pulley drives are located. She was placing the curtain there to keep the door area open for a cool draft and to get some light into the room. As she placed the curtain through the guarded hole in the wall, her right thumb caught the cross belt and pulley. The hole in the wall that gave access to the cross belt conveyors belt and pulleys was only partially guarded. There was a 2-in. by 6-in. opening to the left of the guard. The employee suffered a laceration and a fracture to her right thumb. She was hospitalized and treated for lacerations.
