Investigation Summary
On April 2, 2004, Employee #1 was operating an all terrain vehicle (ATV) while carrying a coworker. The ATV was designed to carry only one rider. For some reason, the vehicle overturned and Employee #1 was struck by the ATV's handlebars. Neither Employee #1 nor his coworker was wearing a helmet. Employee #1 was hospitalized and treated for multiple facial injuries, one of which included loss of vision in one eye. He was released fourteen days later.
