Investigation Summary
On September 11, 2003, Employee #1 was using an 8 ft tall ladder to access the roof of a building. He was planning to secure the ladder from the top using clamps. Employee #1 was stepping from the ladder onto the roof when the ladder shifted and he fell approximately 10 ft to the groud. Employee #1 sustained two fractured wrists and a partially fractured skull. He was hospitalized for four days.
