Employee's Finger Tip Is Amputated When Caught in Chain

On July 10, 2009, Employee #1, an employee of a temporary labor provider, was assigned to change the sprockets on a box conveyor on the client's production floor. He finished the sprocket installation and checked the proper operation of the conveyor. Without turning off the conveyor, he started to install the guard for the sprocket and chain. While he was positioning the guard, his hand went behind it and the moving chain caught and amputated the tip of his right index finger.
