Investigation Summary
Employee #1 works as a cook for a company that operates a concession trailer at fairs and other events. Employee #1 was barbecuing tri-tip steaks at the time of his injury. Employee #1 was standing beside the barbecue when the propane hose on a burner came off the fitting on the burner handle. The propane had been left in the on position. This allowed pressure to build up and blow the hose off its fitting. Employee #1 reported that the propane hose had come off a week earlier. Employee #1 and another employee had pushed it into place and tested for leaks. Employee #1 stated it didn't leak propane so they continued to use the burner. At the time of the accident, the propane hose was whipping around as propane came out of the loose end of the hose. A fire ignited and Employee #1 stated a ball of fire came at him and he jumped over a stack of wood to get out of the way. Employee #1 received second degrees burns to 20 percent of his body, mostly on his face and arms. Employee #1 was hospitalized.
