Investigation Summary
On November 2, 2001, Employee #1, a carpenter, was installing siding on the balcony of an apartment as per instructions from his supervisor, the company owner. Employee #1 was climbing a 24-foot extension ladder, while holding on to a 4 by 4 in. piece of 0.5 in. by 8 in. OC DF siding. He was going to set the siding on the balcony, climb onto the balcony then install the siding while working/standing on the balcony. The deck of the balcony was 12 feet by 6.5 feet and was 9 feet off of the ground. The height of the balcony wall was 4.5 feet. At some point, while placing the plywood on the balcony, with his head close to the top of the balcony wall and his feet just below the deck, he lost control of the siding. As he tried to regain control, he fell backwards nine to ten feet onto the concrete walkway/sidewalk. Though the ladder did not fall or move, Employee #1 lost his balance and fell backwards. Employee #1 suffered a dislocated and fractured hip and shoulder and was hospitalized.
