Investigation Summary
On April 24, 2001, Employee #1 broke his ribs and punctured his lungs when he fell twenty feet from a guardrail on a bridge construction project. Employee #1 was finishing work on the concrete saffet at interstate 80 in Davis, California. He had been working on two different levels of the bridge saffet. Both levels had guard rails and mid rails. The top level was measured at twenty six feet above the ground and the lower level was measured at twenty feet above the ground. Employees use a ladder to go from one level to the other. Employee #1 finished his work at the top level of the saffet. He climbed down to the lower level. He was planning to hard trowel the lower saffet and spray it with cure. However, he left the cure spray can at the top level. While at the lower level, he stepped up on the concrete waler, which was two feet above the platform and grabbed the next waler, which was four feet above the platform, with his left hand. He then reached with his right hand to grab the 5-gallon spray can. Employee #1 slipped, fell backwards, hit the top of the guardrail, and fell over the rail twenty feet to the ground. Employee #1 suffered a punctured lung and broken ribs. Employee #1 went to a local hospital for treatment.
