Investigation Summary

Investigation Nr: 170626774
Event: 05/22/2001
Employee Injured When Struck By Falling Load Of Pipe

At approximately 7:00 a.m. Employee #1 began tightening down a load of PVC pipe at the rear end of the front flatbed trailer. Several loads of PVC pipe had been loaded on the trailer the night before. As Employee #1 was tightening down the load, it appears that one lift of PVC pipe (6 pipes banged together) had been placed on top of the belly strap. As Employee #1 continued tightening the load, one lift of PVC pipe began to lift upwards as the strap became taut. At this point the PVC pipe slid off of the trailer and on to Employee #1, bouncing several times on his back. Employee #1 sustained multiple injuries that included a fractured pelvis, torn diaphragm, fractured ribs, punctured lungs and other internal injuries. Employee #1 was hospitalized.
