Investigation Summary
On July 10, 2001, Employee #1, a foreman in the reinforcing-bar fabrication shop at Pacific Coast Steel, Inc., was using a bridge crane to unload bundles of reinforcing bars from a flatbed truck. When he lifted the second set of bundles, the remaining bundles slid off their support and hit his left foot. Employee #1 was hospitalized with a fractured ankle.
