Employee's Thigh Lacerated by Machine

At approximately 11:30 p.m. on November 2, 2001, Employee #1 was working on the Zecchitte Palletizer Number 9, making an adjustment to the photo eye of the bottle placer mechanism. The Zecchitte Palletizer Number 9 has a "cage" around it, with a door, to prevent anyone from entering the machine while it is in operation. If the cage door is opened for any reason, the Zecchitte Palletizer Number 9 shuts down. The operator's panel has a master shut off switch. Once the master shut off switch is activated, the operator needs to manually reset three buttons to start up the Zecchitte Palletizer Number 9. On the night of the incident, the maintenance employee had the operator open the cage door and activate the master shut off switch. During the photo eye adjustment period, the maintenance employee shut the cage door and told the operator to activate the machine. When the machine turned on, the lever cut into Employee #1's left inner thigh. The employee was hospitalized.
