Investigation Summary
On October 18, 2003, Employee #1 was loading Styrofoam containers on a flatbed truck. He was positioned on top of the bed of the truck approximately 4.25 feet high, without guardrails. As he received lightweight boxes to stack on top of the truck bed, he lost his balance and fell to the ground. He sustained multiple fractures and was transported to the hospital, where he was admitted for treatment.
