Investigation Summary
On July 17, 2001, Employee #1, a clean-up person, was working at a saw mill and was injured during clean up work at the head-rig. The head-rig was the main saw that breaks logs into lumber. Employee #1's job required him to clean up at the saw during breaks and lunch hour. Employee #1 was in the conveyor. He had cleaned up the upper level by the saw and began to clean up the base of the saw floor. Employee #1 removed the lockout pad locks and turned on the control switches to the carriage drive and saw blade. Employee #1's left ankle was caught by the conveyor when the machine turned on, and he was pulled into the conveyor. Employee #1 was hospitalized, where he was treated for a compound fracture of his left ankle.
