Investigation Summary
At 8:50 a.m. on June 16, 2000, Employee #1, of the Lahontan Golf Club in Truckee, CA, was removing dead fish from a water hazard on a golf course. He was wading out when he stepped into a hole and slipped under the surface. Employee #1's coworkers dove into the murky water to try to save him, but they could not find him up in time and he drowned.
