Employee's fingertip amputated after grate falls on it

At approximately 12:10 p.m. on September 28, 2000, Employee #1 was working for Premier Valley Foods. The employee was using a screwdriver to lift a floor drain grate, when the grate slipped and fell on his right ring finger. Employee #1 received a serious laceration and was taken by ambulance to the hospital, where he was treated and released.
