Investigation Summary

Investigation Nr: 170763627
Event: 10/04/2002
Employee killed while repairing a Bobcat front end loader

On October 4, 2002, an employee was repairing a Bobcat front-end loader in the company's maintenance shop. He raised the bucket arm with hydraulic pressure, but failed to extend the safety arm to support the bucket arm before working on the hydraulic system. He positioned his body between the frame and bucket arm to perform the work. When he loosened a hose on the hydraulic system, the bucket arm came down and crushed his upper chest and shoulder area. The employee was killed.
