Investigation Summary

Investigation Nr: 170873723
Event: 03/20/2002
Employee injured after struck by the beam

On March 20, 2002, Employee #1 and a coworker, welders of Butler Manufacturing Inc., were working on a 40-ft-wide structural beam putting some finishing welds on it. They had lifted the beam to the vertical position and set it on saw horses. As they were walking to their welding positions, the beam started to tip over. The lifting clamp was either loose or had come off, and the beam struck Employee #1 causing a fractured pelvis. He was taken to Kaweah Medical center for emergency treatment then transferred to U.M.C. where he underwent surgery.
