Investigation Summary
On August 29, 2008, Employee #1, employed by a subcontractor, and the company foreman were installing metal flashing on a northeast area of a roof, at a construction site. The employees needed more flashing material, and they exited the roof via the roof hatch and fixed ladder on the southwest side of the roof. Employee #1 then stood on the roof, next to the roof hatch, while the foreman stood on a platform halfway up the fixed ladder, to hand him the metal flashing. As Employee #1 was pulling up the bundle of metal flashing, he stepped backward, onto the skylight. When Employee #1 stepped on the skylight, it broke, and he fell through the skylight, to the concrete floor, 25 feet, 9 inches below. Employee #1 was seriously injured, sustaining compound fractures to his legs, feet and ankles. He was hospitalized for over 30 days. The employer received citations related to unprotected skylights.
