Investigation Summary

Investigation Nr: 201105632
Event: 07/20/2004
Employee's Fingers Are Amputated by Machine

At approximately 6:00 a.m. on July 20, 2004, Employee #1, a production machine operator at the Number 2 hopper-mixer, removed the cover plate at the bottom of the conveyor line to clean out product that apparently had jammed the machine. Since the belt was not in motion, Employee #1 did not shut down the unit. As Employee #1 cleaned out the product using his right hand in a sweeping motion, the jam cleared and the machine began running. His right hand caught between the turning conveying auger and the conveyor frame. Employee #1 was hospitalized for treatment of amputated fingers.
