Employee Is Injured after Tripping and Falling in Elevator

At approximately 7:00 p.m. on February 29, 2004, Employee #1 was making food deliveries using a dietary elevator. Employee #1 stopped on the 4th floor. At this time, Employee #1 had noticed that the threshold entry/exit area was uneven. Employee #1 stepped out of the elevator, leaving the dietary cart in place and made the food deliveries. As Employee #1 stepped back into the elevator, Employee #1 tripped and fell, sustaining a serious non-reported injury. As a result, hospitalization was required.
