Investigation Summary
Employee #1, a drill operator, was underneath the bridge standing on a platform that was 8 ft by 8 ft and 3 ft above the water. In the middle of the platform was a 60-in. diameter concrete pylon. The concrete pylon had a steal ring around it that was elevated above the platform. The steal ring was thick, and was approximately 4 in. above the surface of the platform. Sitting on the platform were two 8-in. by 8-in. by 4-ft timber used for standing while working next to the pylon. Employee #1 was standing on the 8-in. by 8-in. timbers and had just finished bailing out (cleaning the area out) the section of pile and was going to check the elevation of the bore. Employee #1 stepped off of the stacked timbers (16 in. high), tripped, and fell toward the concrete pylon. Employee #1 struck his right kneecap on the steal ring breaking the kneecap. Employee #1 was hospitalized.
