Investigation Summary

Investigation Nr: 201175858
Event: 05/05/2004
Employee is hospitalized after fire extinguisher strikes him

On May 5, 2004, a service technician for a fire extinguisher company delivered a large fire extinguisher to the shop for servicing. The employee was standing near the door as the hydro tester began to operate. The service technician who was not familiar with this type of extinguisher pushed the firing pin, thinking that it was the pressure release valve. The extinguisher expelled its contents into the room, and the extinguisher struck the technician as it spun around on the cart. His injury required hospitalization for an abdominal tear.
