Employee is crushed and killed while repairing forklift

At approximately 5:10 a.m. on March 11, 2004, Employee #1, a mechanic working for Mary Anne's Baking Company, Inc., was replacing the starter motor on a forklift. The starter motor was located under the frame of the forklift. To gain access, the employee placed a hydraulic jack under the counter weight at the back of the forklift and jacked up the forklift. He then slide under the forklift and began removing the starter motor. The jack slipped out from under the forklift and the forklift fell onto the employee's head, crushing and killing him. The counter weight had a rounded bottom and there were no blocks or supports used to keep the jack from slipping.
