Investigation Summary
On September 2, 2004, Employee #1 and a coworker, both truck drivers, were delivering car batteries to a business at its shipping and receiving dock. The vehicle that contained the batteries was a semi-tractor trailer rig. Employee #1 and his coworker arrived at the site before the business was open. They backed up the rig into the loading dock, leaving a little less than 3 feet from the loading dock platform. When the business opened, Employee #1 got out of the cab of the vehicle and walked to the back of the trailer to break the seal on the trailer and open it up to unload cargo. While attempting to open the trailer, the coworker backed the rig up to get closer to the loading dock. Employee #1 was pinned against the wall of the shipping receiving loading dock. He was hospitalized and treated for a fractured pelvis. During the accident investigation it was revealed that the coworker failed to follow administrative controls to ensure Employee #1 was clear from the rear of the vehicle. Additionally, the employer was cited for failure to report a serious injury (T8, 342 (a)).
