Investigation Summary
Employee #1, a production leadman working for a company that manufactured fiber glass insulation, was re-feeding a roll of plastic poly wrap into the roll-up machine. He reached under the guard at the point of operation without stopping the feed rolls and the tips of his fingers were crushed in the rolls. The employee was able to pull his fingers free from the rolls but the tips of his right middle finger and right ring finger were amputated.
