Investigation Summary
On April 1, 2003, Employee #1 works for the company as a millwright. He was placing a chain back on a sprocket. He locked-out a drive unit at the location he would be working. However, he did not lockout the actual chain he was placing back on the sprocket. He had both hands on the chain pulling slack out of the chain so he could place it on the sprocket. Employee #1's hand was pulled into the sprocket and the tips of his little and ring fingers were amputated. He was not hospitalized.
