Investigation Summary
Employee #1, a grounds keeper for a resort in Shasta Lake, CA was raking leaves and performing clean-up work at the edge of a parking area. The parking area was on a slight slope and a pickup truck that was parked at the warehouse rolled down the slope and struck the employee. He was run over and killed. The employee had his back to the truck and apparently did not see or hear the vehicle approach. The vehicle's engine was not running at the time of the accident.
