Investigation Summary
At approximately 8:00 p.m. on December 15, 1999, Employee #1 was unloading milk products from a truck using a ramp to bridge the space between the loading dock and the vehicle. He was pulling a cart filled with about 300 lb of goods onto the ramp when the wheels of the cart dislodged the ramp from the truck. Employee #1 and the cart fell about 4 ft, with the cart landing on top of him and cutting off his breathing. He died of positional asphyxia. The loading ramp was not secured.
