Employee's hands and foot caught in press machine

At approximately 10:00 a.m. on September 4, 2001, Employee #1, a feeder and pressman assistant, loads paper into a printing press, a Lithrone 40, Model L-540, Serial Number 119, Manufactured by Komori Printing Machinery Co., Ltd. The press consists of six rollers above the work surface that are approximately 48 inches long. The rollers are stacked one above the other, approximately 31 inches high. The pressman and Employee #1 noticed small pieces of paper were on the rollers. The pressman went to the off load end of the press to check the printed product while the printing press was still operating. Employee #1 attempted to remove a tiny piece of paper from one of the rolls with a small rag, approximately 8-in. square, and folded into his right palm. He intended to dab at the piece of paper as it passed by when his right hand was drawn into the in-running rolls between the copper roller and the form roller. With his left hand he grasped his right wrist trying to free it. His left hand was then drawn in between the rolls. While trying to free his hands, his left foot was drawn in between the plate and blanket rollers. Employee #1 was hospitalized with multiple hand injuries and an amputated foot.
