Investigation Summary
On October 5, 1999, Employee #1, a rice polisher for a company that dries, mill, stores, and packages rice, was straightening the shroud on a rice bran discharge chute when his right hand became caught in a rotating air-lock valve. Employee #1's thumb was amputated at its mid-joint and his index and middle fingers were amputated at their distal joint. The air-lock valve was not guarded.
