Investigation Summary
On July 22, 1999, Employee #1 was using a disc sander to remove a sharp burr from the edge of a 4 in. square metal plate. His left thumb became caught between the metal plate and the sanding disc, amputating the tip. Employee #1 was not able to explain how the accident occurred.
