Investigation Summary
On July 16, 1999, Employee #1, an equipment operator and cleanup person for a cheese manufacturer, was reaching up into the drop outlet of a screw conveyor to check for cheese residue when her right hand was amputated. A coworker had told her that he had locked out the machine, but it was still operational.
