Employee's arm fractured when crushed against forklift
On January 18, 1999, Employee #1 was helping to upright a 3 in. steel pipe roof support that had been knocked over by a forklift when his left arm became caught and was crushed between the pipe and the mast of the forklift. He suffered fractures to both bones in his forearm.
