Employee's fingers are amputated when caught in belt

At 7:00 p.m. on July 28, 2017, Employee #1 was loading plastic onto a conveyor belt when the grinder became jammed. The employee attempted to remove the jam in the machine by manually moving the unguarded belts. Employee #1's hand became caught in the belt and pulleys, amputating three of his fingers.
