Employee injured in fall from rebar wall

On August 25, 1998, Employee #1 was working on a rebar wall during construction of a wall bulkhead. He fell between 13 and 16 ft, landing on his feet on a trough guard over vertical rebar. He then fell 2 1/2 ft to the concrete floor. He sustained serious injuries, for which he was hospitalized. Employee #1's body harness and short lanyard were being used for a positioning device system. His attachment hook apparently pulled off the end of the horizontal when the rebar ties to the vertical rebar broke.
