Investigation Summary
On May 5, 1998, Employee #1 was on cleanup detail in the basement of a sawmill, situated under a new small log operation. He was shoveling up wood debris when a 2 ft long round fell either from a nearby gang incline conveyor or from a floor opening on the mill deck above, striking him near his right hip. About a week later, Employee #1 experienced pain in that area and had an MRI, which revealed a fractured L4 vertebra. He underwent surgery during the first week of July.
