Investigation Summary
On September 30, 1998, Employee #1 was helping coworkers replace a bar conveyor on a walnut pickup machine. He was pulling on the conveyor as power was applied to the drive sprocket. The heavy conveyor suddenly moved around the sprocket and doubled back, catching and amputating the tip of his right middle finger just above the distal joint.
