Investigation Summary
On October 24, 1998, Employee #1, of Adecco, was engaged in clean up work for Pacific International Rice Mills, Inc. He was riding a man lift in a rice dryer when part of his backpack blower apparently became caught on the metal shroud leading into a floor opening and deck. Employee #1 was pulled off the man lift and fell 31 ft onto the concrete floor. He was killed.
