Investigation Summary
On September 18, 1998, Employee #1, a sawyer for a company that manufactures wood trusses, was working with an Irvington cut-off saw. When the 2 x 4 suddenly stopped, his hand slipped and struck the turning 18 in. blade. Employee #1's left index finger was amputated to just above the base. The guard on the saw was not properly adjusted.
