Investigation Summary
On December 12, 1997, Employee #1 was checking to see why descaling chemical was not feeding through a fuel additive hopper. He was using a short piece of flat conduit with a bolt/nut on the end to clean the small hopper, and had reached inside when the shaft caught the cuff of his leather glove. His right hand and arm wound around the shaft, fracturing his wrist and forearm. The shaft was slow moving, but it was not guarded.
