Employee's fingers amputated in V-belt and pulley drive

On August 7, 1997, Employee #1, a laborer at a prune dehydrating facility and who spoke only Punjabi, was trying to clear a jammed prune tray cart. His left hand swung into the nip point of an unguarded V-belt and pulley when he kicked the cart, amputating the tips of his index, middle, and ring fingers. This was the actual cause of the accident, as told in an interview on September 4, 1997, with Employee #1's son acting as an interpreter. On the day of the accident, Employee #1 was told by his supervisor, who interpreted for him, to say that he was replacing a V-belt that had come off. Employee #1 said that he was stretching the new belt and rotating the pulley when the tips of his fingers became caught and were amputated between the belt and pulley.
