Employee's thumb amputated when pulled into chop saw

On September 2, 1997, Employee #1, who worked for a remodeling contractor, was cutting a groove in the end of a short piece of 4 to 6 in. long by 4 1/4 in. wide by 3/4 in. thick oak flooring by "free handing" on the bottom of the chop saw blade. His left thumb was pulled into the blade and amputated.
