Investigation Summary

Investigation Nr: 201100476
Event: 09/28/1997
EMPLOYEE INJURED IN FALL FROM LADDER

The employer operates a variety and drug store at this location. Employee #1 worked as an associate of the store doing various clerk duties. Employee #1 fell from a ladder as he was attempting to step onto it from a loft that is atop a break room in the storage area of the store. This loft area is 42 inches high and was being used to store spare shelving. Employee #1 had climbed up to the loft to get a peg for a display rack in the Camera Department. Employee #1's foot slipped as he put it back on the metal ladder as he crawled off the loft area. The ladder conforms to the ladder requirements of the General Industry Safety Orders. There is no platform at the loft, as the ladder is for access to the roof, not the loft. Employee #1 received a fracture to his right wrist and a shoulder and foot injury.
