Investigation Summary

On December 9, 1997, Employee #1 died of asphyxia when crushed by a mast of a forklift, which squeezed him into the cab of the lift truck. Employee #1 was between the mast and the cab when he sustained an injury to the chest. Employee #1 worked as a truck driver for the employer who operated a saw mill at an Anderson, CA location. His job was to move lumber from the Anderson mill to a mill located in Redding, CA, three miles away. To do this job, Employee #1 on numerous occasions drove a forklift truck and loaded his truck with lumber. It looks like Employee #1 was cleaning the top window glass at the time of the accident and while doing so he made contact with the tilt lever. The lever was activated and caught Employee #1. Employee #1 had many years of experience operating trucks and forklifts.
