Investigation Summary

Investigation Nr: 201100583
Event: 05/06/1998
EMPLOYE STRUCK IN THE CHEST BY A TAIL STOCK

Employee #1, a welder, was struck in the chest by the tail stock as he was reaching across the tail stock to adjust the air valve. His employer manufactures steel tanks of all sizes. Employee #1 had finished forming metal into the rounded form for a tank in the forming rolls. He was opening the rolls to remove the metal tank side when the forming rolls bound up. Without thinking, he reached across the tailstock to move the air valve to release air on the tail stock so it would come unbound. The tail stock holds the forming rolls together. When the tail stock is moved away from the rolls, material can slide between the rolls. As he reached over the tail stock, it struck him in the chest when it moved suddenly after it came unbound. He stated he knew better than to reach across the tail stock. He was hospitalized for several days for observation because of concern of being hit in the chest near his heart. The impact caused bruises on his chest.
