Investigation Summary
Employee #1 worked for a small wood products manufacturing company that made futons. He was using a 12 in. table saw to rip a 1 by 4 by 14 in. into 1 by 1 1/2 by 14 in. Employee #1 made a cut by pushing the piece of wood through the saw. He was reaching out to pick up the pieces when the wood section struck the unguarded saw blade. The wood was pulled backward, carrying Employee #1's finger across the turning blade. His left ring finger was amputated down to the first joint.
