Investigation Summary
On May 10, 2006, Employee #1, worker at a department store, was operating an industrial forklift. As he was maneuvering the forklift, he placed his left leg on the outside of the forklift. He misjudged the close proximity of an I-beam and crushed his leg between the forklift and the I-beam. Employee #1 was taken to a local hospital, where he was hospitalized and treated for a compound fracture to his left leg.
